Inside Dementia Home Care San Diego

LEARN MORE ABOUT OUR MISSION

History

Our History
Dementia Home Care San Diego was founded with a simple yet powerful vision: to provide compassionate, specialized in-home care for individuals living with dementia and Alzheimer’s disease. Our journey began over a decade ago when a local caregiver, inspired by personal experience caring for a parent with memory loss, recognized the urgent need for high-quality, dementia-focused home care in San Diego.
From a small team of dedicated caregivers, we have grown into a trusted care provider known for professionalism, empathy, and a personalized approach. As awareness of dementia-related illnesses increased, so did our commitment to training, innovation, and education. We introduced specialized programs for sundowning dementia care, dementia wandering supervision, and advanced dementia home care, ensuring families could find comfort and confidence in our support.
Today, we proudly serve clients across San Diego County, helping them age in place safely with the help of our certified dementia caregivers and professional memory care caregivers. Our history is rooted in real families, real challenges, and a relentless passion to improve lives. We honor that legacy every day by delivering care that goes beyond tasks—it brings peace, dignity, and a better quality of life to those who need it most.

About Us: From Crisis to Calm

We help you restore safety to your loved one and balance to your life.

Replacing Crisis with Calm.

Our mission is to replace the uncertainty of dementia with the confidence of expert care. We provide specialized, proactive in-home support that protects the safety and dignity of those with memory loss, while restoring balance and peace of mind to the families who love them.

When Waiting Is No Longer an Option

Many families reach out to us not because they planned to, but because they realize—often suddenly—that their loved one can no longer be left alone safely. When memory loss begins to affect daily life, waiting often leads to a crisis that forces rushed decisions.

At Cognihealth, we help you take timely, compassionate action. We put reliable, state-certified in-home support in place before risks escalate, allowing you to step back from the role of “crisis manager” and return to being a son, daughter, or spouse.

If you are caring for a parent with dementia, you know that love and effort alone cannot prevent wandering, falls, or nighttime agitation.

You may be balancing caregiving with a career, raising children, and managing your own health. The constant vigilance, disrupted sleep, and “always-on” anxiety can leave you stretched thin. You do not have to carry this alone.

We provide state-certified, background-checked, and specifically dementia-trained Home Care Aides. Our goal is to stabilize your loved one’s routine and reduce risk. We provide the critical support you need, including:

  • Safety & Supervision: 24-hour monitoring or overnight care to manage wandering and anxiety.
  • Personal Dignity: Hands-on assistance with bathing, dressing, and hygiene.
  • Daily Rhythm: Meal prep, hydration, and light housekeeping to maintain a calm environment.
  • Companionship: Specialized engagement to reduce anxiety and promote calm.

If you recognize your situation in the list below, a conversation now can help you regain balance before a medical crisis occurs. Families usually call us when facing:

  • Physical Exhaustion: You are suffering from sleep deprivation or burnout from being “on alert” 24/7.
  • Escalating Behaviors: Increased confusion, wandering, agitation, or “sundowning” (nighttime wakefulness).
  • Recent Hospitalization: A sudden decline in ability following an illness or fall.
  • Unsustainable Gaps: Realizing that occasional family check-ins are no longer enough to ensure safety.

Our Promise: We help protect your loved one’s safety while preserving your own well-being.

A Message from Our Founder

“I know the weight of this journey because I’ve walked it.”

My commitment to dementia care is both professional and personal. Like many of you, I experienced firsthand what it means to care for a loved one with dementia while trying to balance safety, dignity, and day-to-day life.

Supporting my mother through her dementia journey gave me a deep understanding of the difficult decisions families face. I know the guilt, the exhaustion, and the fear of making the wrong choice.

I created this program to remove that uncertainty for other families. We focus on clear guidance, consistent caregivers, and care plans that adapt as the disease progresses. With us, you are never managing alone.

Our Core Values

1. Proactive Protection We don’t just watch; we anticipate. In dementia care, safety relies on staying one step ahead. We look for risks before they happen—from fall hazards to signs of agitation—so we can address them before they become emergencies.

2. Unwavering Dignity We see the person, not just the diagnosis. Whether assisting with bathing or managing confusion, we treat your loved one with the utmost respect, preserving their independence and self-worth in every interaction.

3. Compassion Born from Experience We understand because we have been there. Our care is driven by a deep, personal understanding of the emotional weight families carry. We meet every challenge with patience, empathy, and a calm presence.

4. Reliability You Can Lean On When you are exhausted, uncertainty is the enemy. We provide consistent, dependable caregivers who show up on time and prepared. We are the stable foundation you need to regain balance in your own life.

5. Transparent Partnership You should never feel in the dark about your loved one’s care. We believe in open, honest communication. We work with you, keeping you informed and adapting the care plan as your family’s needs evolve.

Getting Started: A Simple Path to Relief

Reaching out for help is often the hardest step. We make every step after that simple, transparent, and supportive.

Step 1: The Initial Conversation (We listen before we lead) When you call us, you won’t get a sales pitch. You will speak with a care coordinator who understands dementia. We will listen to your current challenges, discuss the behaviors you are seeing, and answer your immediate questions.

Step 2: The In-Home Safety Assessment (We come to you) If you decide to move forward, we schedule a complimentary in-home visit. We meet you and your loved one in their comfortable environment to identify safety risks and learn your loved one’s daily rhythm.

Step 3: The Custom Care Plan & Caregiver Match (We find the right person) Based on our assessment, we select a caregiver whose skills and temperament match your loved one’s needs. We build a detailed Care Plan that outlines routines and strategies to manage anxiety or confusion.

Step 4: The Introduction & Ongoing Support (We stay involved) On the first day of care, we are there to ensure a smooth introduction. We monitor the care logs, adjust the plan as the disease progresses, and ensure you always feel supported.
